I would assume you are speaking about Honda transmissions and the answer would be, of course they do. The four speed manual transmission will have longer gear ratios so that you can attain close or equal to the same maximum speed of the five speed gear box. In doing this, your acceleration will be slower with the longer geared 4 speed versus the shorter geared 5 speed.
Not the best of descriptions, but gets to the point.
4 speed does not necessarily mean you are slow. You would get better gas mileage with a 5 speed though. 4 speed automatics may be slower than manuals. It would affect your top speed mostly.
