Default TAXES
If I didn't file a return for 2003, can I do it for the 2004 return? If so, do I just use the 2004 form or do I need to get the 2003 form?

Yes, you can just use a 2004 tax form and file all your W-2's from both years on there. I believe the same goes for state too.
